최종덕, "의철학 3대 논쟁점의 재구성", 의철학연구 20 (2015), 125-176쪽 【주요어】 의철학 교과서, 의학적 존재론, 질병 정의, 의철학의 고유성, 생의학 모델과 인본의학 모델【요약문】의철학의 내용은 의학의 문제를 철학적으로 해석하는 주제영역과 의철학의 기본적인 논리와 범주를 다루는 기초영역을 포함한다. 이 논문은 이 중에서 의철학 교과서의 내용 중에서 한 축인 기초영역에 대한 것이다. 의철학의 기초영역 부분은 지나온 40년 동안 부어스. 펠리그리노, 카플란, 커닝엄, 울프와 슈바르츠, 카셀과 잉겔하르트, 마컴과 킹 등 대표적인 의철학자들에 의해 연구되어 왔다. 이 연구논문의 목적은 그 많은 기초영역의 연구들을 의철학 교과서에 어울리도록 재구성하는 데 있다. 필자는 기초영역을 1) 의철학의 고유성 여부 논쟁, 2) 질병 정의에 대한 논쟁, 3) 인본의학 모델의 가능성 논쟁의 이론들로 재구성했다. 첫째 이 연구는 의철학이 고유한 영역으로 가능하지를 묻는 지나온 논점들을 정리해서 다루었다. 둘째 의학적 존재론에 해당하는 것으로서 질병에 대한 여러 가지 설명과 규정, 혹은 정의에 대한 많은 논쟁점을 다양한 관점에서 객관적으로 전개했다. 셋째 의학적 가치론과 인식론을 포함한 논점으로서 과학주의 의학과 인본주의 의학 사이에서 그 차이와 통합가능성을 다루었다. 이 연구가 의철학의 새로운 논점을 창조적으로 제시하거나 기존의 논쟁점을 새로운 방식으로 해결하는 것은 아니다. 단지 기존의 논점들을 새로운 범주로 재구성함으로써 의철학 인식체계의 지식지도를 제공하고자 기대한다. In my view, the philosophy of medicine can be composed of the topic-centered domain, which focuses philosophically on the practical problems of medicine, and the normative domain, which deals with foundational theories and methodological thesis in medicine. My paper concerns just a normative domain of the philosophy of medicine. The so-called normative domain contains three sorts of problems of philosophy of medicine; the first point about whether the philosophy of medicine has its own and irreducible field, the second issue over how to define disease, and the last problem of disputes between the biomedical model and the humane or humanistic model. The normative domain including the diverse philosophical arguments in medicine already has been covered for the past 40 years by Wulff, Schwartz, Pellegrino, Marcum, King, Cunningham, Huneman, Douglas, Engelhardt, Boorse, Caplan, et cetera. My article is not to solve a disputable problem or not to suggest new issue additionally in the philosophy of medicine, but to commit a reconfiguration of the existing arguments in order to organize these diverse arguments in an easily accessible manner. I, therefore, tried to figure out the epistemological map of the philosophy of medicine, which could find a way to generate a good text on the philosophy of medicine in Korean knowledge territory.
